WAIVER FORM

FYI - This link is primarily here for those that need parent's signature.  You can fill it out ahead of time, or at the field. We'll have plenty of waiver forms at the field when you get there.   

PRINTABLE REGISTRATION FORM

This form is for teams and individual players.  If you're not on a team, just leave the team info blank and you will be listed on our check-in list under your last name.

BEFORE or AFTER the deadline: 
You can save $10 on entry by pre-paying for your entry two weeks prior to the game.   

Under both "Entry" and "Air," there are two numbers for the father/son discount [e.g., adults pay $50; kids with playing parent pay $25].

*NOTE:  The online form is JUST the "paperwork" portion of it.  Filling this out does not take out money from your PayPal account, does not write us a check and does not give us a credit card number to process.  This form is JUST so that we can link up the PayPal receipt/check/credit card information with your registration paperwork.

If you want to let us know you're coming, please use the online form. 
If you're mailing a check, mail the registration form with the check. 

* Please note that you have to pay your Entry Fee prior to the two week deadline in order to get the $10 entry fee discount.  You can pay for entry, paint and air all before the game, but only the entry fee is subject to the $10 savings for doing so.  Don't come up and say "I posted on the forums that I was coming two months ago, so I should pay $10 less."  This is a PRE-PAYMENT discount. 
 

PAYPAL[?] INFORMATION (OPTIONAL)

Before the game... You can pay by check, money order, or online with a credit card using PayPal [?].  PLEASE don't mail anything after one week before the game, and don't make a PayPal purchase later than two days before the game [unless you just really don't want us to be able to verify the purchase].

At the field... we take cash and checks only.  IF you have a PayPal account, we can take your info there, but it WILL be a bit of a hassle, relative to cash or check. We will have a backup receipt of your check/money order/PayPal transaction, should you need a copy.

AIR: (Covers All-Event Air, N2 and CO2 available)

	Youth Air Fee (Ages 10-15, with playing parent): $7.50
