FYI - This link is primarily here for those that need parent's signature. You can fill it out ahead of time, or at the field. Yes! We have waivers at the field for you to sign.

This form is for teams and individual players. If you're not on a team, just leave the team info blank and you will be listed on our check-in list under your last name.

*NOTE: The online form is JUST the "paperwork" portion. Filling this out does not take out money from your PayPal account, does not write us a check and does not give us a credit card number to process. THUS, this is not "PRE PAYING" for anything. This form is JUST so that we can link up the PayPal receipt/check/credit card information with your registration paperwork. Instructions for paying are below.

If you're mailing a check, mail the registration form with the check NO LATER THAN one week in advance of the game. DON'T send something in a day before the game and ask why we didn't receive it.

Below this online registration link, you'll find the PayPal "shopping cart" items. Click on entry/air, paint (if needed), then hit the Check Out button. Simply Increase the number of admission fees, etc. from the check out screen to match the registration form. Without the online form we will not know what paint/admission goes with what people.

PAYPAL? INFORMATION (OPTIONAL)

Before the game... You can pay by check, money order, or online with a credit card using PayPal [?]. PLEASE don't mail anything after one week before the game, and don't make a PayPal purchase later than two days before the game [unless you just really don't want us to be able to verify the purchase].

At the field... Doom takes cash and checks only. Battle Zone takes cash, checks and credit cards.

We will have a backup receipt of your Check/Money Order/PayPal transaction at the field, should you need a copy.
